Remember that you must take a signed hardcopy of this checklist with you on the expedition and hand it over to your expedition leader at the start of the expedition. Further instructions on how this works are in the checklist itself and also in the dossier that we will have sent you when you signed up. If you do not have this dossier or if you do not know what it is, then please get in touch.

Please make sure you bring an up-to-date copy (fewer than six months) of this statement with you. Even though you will already be a qualified diver of PADI or another dive organisation, you still need to complete the PADI medical statement and bring it with you. Please note that under certain circumstances (explained on the medical statement), you need to see a doctor before the medical statement is complete, so give yourself enough time to do this.
